Licensure and Your Career

Transcript Request Form (Form 122)
Send this form to your educational institution to request your transcripts be sent NCARB. NCARB cannot accept a student-issued transcript, it must be sent directly from your college or university. If you were educated outside of the United States or Canada, do not use this form. Contact NAAB to have your education evaluated.

Updated! The IDP Workbook is a tool for interns to use to help calculate their training hours. The workbook can also be used to track your progress through the IDP in addition to the tools in the e-EVR.

IDP Reporting Extension Request Form
Form to request an extension for the Six-Month Rule. Extensions are granted for the birth or adoption of a child, serious medical conditions, active duty in military service, or other such like causes.
